If you are in need of a new or updated medical, Special Olympics New Jersey will be hosting free medical screenings for all registered athletes on Saturday, January 6 at Winter Games located at The Seaview Hotel. Register below!

Before arriving, you must complete the following forms:

  1. Online medical form through DocuSign.
    Once completed through DocuSign, you do not need to bring any paperwork with you. To find out your area please click HERE.
  2. Communicable Diseases Waiver
    This form is valid for 3 years.
