If you are in need of a new or updated medical, Special Olympics New Jersey will be hosting free physical examinations for all registered athletes on Saturday, September 23, 2023 at the RISN Center from 10:00am – 2:00pm. Register below!

Before arriving, you must complete the following forms:

  1. Online medical form through DocuSign. Once completed through DocuSign, you do not need to bring any paperwork with you. To find out your area please click HERE. Please follow these step by step instructions for completing the medical forms HERE. 
  2. Communicable Diseases Waiver Valid for 3 years and should be completed with the athlete medical.
