Here’s a quick list for athletes, coaches and volunteers that contains all the necessary requirements and new waivers.
Athletes:
- Current Participation Packet (Medical) – Expires every three (3) years
- Communicable Diseases Waiver – Required once per participant
Coaches, LTP Coordinators, Chaperones and Committee:
**Participants must be at least 18 years old to coach
- Class A Volunteer Registration Form and Background Screen & Communicable Diseases Waiver – expires every three (3) years
- Protective Behaviors Certification – expires every three (3) years
- Concussion Assessment Certification (coaches only) – expires every three (3) years
Unified Partners:
Ages 18+

- Class A Volunteer Registration Form and Background Screen & Communicable Diseases Waiver – expires every three (3) years
- Protective Behaviors Certification – expires every three (3) years
Under age 18

- Minor Class A Volunteer Registration Form – expires every three (3) years
A parent or legal guardian must fill out on behalf of the minor Unified partner.
- Minor Class A Volunteer Registration Form – expires every three (3) years
Event Volunteers:
- Class B Form and Waivers – Expires every three (3) years
